About the role

We are seeking an experienced Registered Nurse who can deliver high-quality, person-centred clinical care to older adults living in the community, aligned with the principles of the Support at Home program. In this role, you will work conduct comprehensive assessments, developing and managing individualised complex care plans, and delivering a wide range of direct clinical services such as medication management, wound care, catheter care (IDC and SPC), diabetes education and stoma therapy.

Salary: $$132,393.12 per annum or $67.00057 per hour plus 25% casual loading, plus 12% superannuation based on skills and experience (pro-rata for part-time employment).

 

Key Responsibilities

  • Apply advanced clinical judgement and initiative in managing participants with diverse/and or complex care needs, to ensure consistent, holistic care that respects each participant’s values, beliefs, and cultural background.
  • Support the intake and onboarding of new participants, including clinical screening, risk assessment, and care transition planning
  • Deliver comprehensive, high-quality, person-centred clinical care in home and community settings, including complex medication management, advanced wound care, chronic disease coordination, and palliative care.
  • Maintain high quality, accurate, timely, and professional clinical documentation in line with organisational and regulatory requirements.
  • Lead communication with GPs and ensure high standards of clinical documentation and records across the care team

What we are looking for

We are seeking a motivated and experienced registered nurse who is passionate about driving continuous improvements and supporting BSL’s mission.

Key skills and experience

  • Bachelor of Nursing or equivalent Registered Nurse qualification with AHPRA along with a current AHPRA Certificate of Registration
  • Post graduate qualifications and experience in a relevant health specialty; continence, wound management, gerontology, infection control, palliative care, dementia care, catheter care (IDC and SPC), diabetes education, stoma therapy, mental health or health management
  • Demonstrated leadership skills with an ability to act as a positive role model for staff
  • Well-developed interpersonal and communication skills with the demonstrated ability to deal with a broad range of people from diverse and socio-economic backgrounds.
  • Demonstrated knowledge and understanding of Accreditation standards, participant rights and best practice principles as applicable to aged care
  • Theoretical and practical knowledge of occupational health and safety regulations and requirements practices, including manual handling, infection control
  • Falls prevention, seizure management and PEG feeding experience
