Brotherhood of St. Laurence (BSL) is looking for a passionate and driven Community Connector to join our collaborative team in Sunshine on a 12-month fixed-term contract.

About the role:
As a community connector you will lead community navigation and engagement with older adults and community members seeking to understand and access community supports. You will act as a point of contact, supporting people to navigate aged care systems including My Aged Care and HACC-PYP, while identifying emerging community needs to inform service planning and improvement.
In this dynamic role, you will:

Design, lead and deliver place-based community engagement activities that build trust, participation and inclusion.

Act as a visible and trusted BSL presence within the community, promoting inclusive community connections.

Support participants to understand and access aged care, community care, Home and Community Care – Program for Younger People ( HACC-PYP) and other relevant services through information, advocacy and assertive outreach.

Identify service gaps and emerging community needs and provide feedback to the Program Manager.
- Assist participants to navigate My Aged Care and relevant assessment services.
Salary: $90,962.92 per annum, based on skills and experience plus 12% superannuation (pro-rata for part-time employment)
