About the role

We are seeking an experienced Care Partner to deliver high-quality, person-centred support to participants within the Support at Home program. In this role, you will work closely with participants, families, carers, and providers to ensure safe, independent living aligned with individual goals, using a strengths-based approach across multiple care pathways. 

Salary: $90,962.92 per annum, based on skills and experience plus 12% superannuation (pro-rata for part-time employment)

Key Responsibilities

  • Be the primary point of contact for participants and their registered support and/or their family members, identifying participant needs, goals, personal preferences, and existing supports, and provide timely information on available services.
  • Review the participant’s support plan and assist the participant to understand their approved services.
  • Refer participants for psychosocial and clinical support to Social Workers, Psychologists and other allied health professionals
  • Communicate with relevant stakeholders involved in the delivery of services regarding the participant’s needs and wellbeing to action the care plan and associated recommendations in a timely manner.
  • Provide options and support to participants within their financial resources, and manage quarterly budgets to avoid underspends/overspends, especially during illness or recovery.
  • Advocate for the participant through the delivery of services that champion wellness and reablement approaches.

What we are looking for

We are seeking a motivated and experienced care partner who is passionate about driving continuous improvements and supporting BSL’s mission.

Key skills and experience

  • Knowledge of home care packages and an understanding of aged care and disability support needs.
  • Experience working with people with dementia, complex ageing-related conditions, or palliative needs (desirable)
  • Tertiary qualification in Health or a related field (including Individual Support – Ageing, Health Services Assistance, Aged Care, Disability, Community Services)
  • Diploma-level qualification in Community Services, Aged Care, Case Management, or equivalent experience
  • Familiarity with My Aged Care portals and digital case management systems (desirable)
  • Well-developed interpersonal and communication skills, with the ability to establish and maintain effective working relationships across all levels both internally, externally and with people from diverse backgrounds
